Patient Information

New to our office? Welcome! You can download and print our New Patient Paperwork to complete before your visit. Or you can fill out the forms in our office.

What to expect during your first visit

Your first visit isn’t only about our dental team getting to know you, but also about you getting to know our team. In the time we spend together, we encourage you to ask questions, express your concerns, and share with us anything you want us to know to help customize your care.

Part of getting to know you and providing safe, personalized dental care involves reviewing your medical history. Certain medical conditions and medications can influence your dental treatment—so you’ll be asked to provide a list of your medications (including vitamins and supplements).

Next, a full-mouth series of dental x-rays and a comprehensive oral exam of your teeth, gums, mouth, and jaw will be performed, including a non-invasive oral cancer screening.

And if you haven’t been to a dentist in a long time, please don’t worry. You’ve made an important decision—no need to feel guilty!

Once your entire exam is completed, one of our dentists will talk with you about their findings. Together, you can create a healthy, effective treatment plan that best meets your dental and personal requirements.

Finally, we want you to know we’ll go above and beyond to make you feel relaxed and safe. Not only is your physical comfort important to us, but your emotional comfort is as well. So, come in, recline and unwind—you’re in great hands.

Insurance and payment

We accept the following forms of payment: Visa, MasterCard, Discover, American Express, cash, and personal checks.

We also participate with multiple insurance plans. If you want an in-network provider, we recommend you contact your insurance provider to verify that information. Though dental insurance is a private contract between you and your insurance company, we are happy to file your claim. We will fully attempt to help you receive full insurance benefits; however, you are personally responsible for your account.

We are pleased to offer financing through CareCredit.
